這是一位好心的讀者提供給我的, 我覺的十分地有用, 所以決定把它分享給大家. 通常你去看病時, 醫院會要你填一份如下面的病歷表, 很多人平常簡單的英文都不會了, 更不用說這些醫學名詞了.? 我把所有的英文都加上中文解釋, 各位可以對照著查詢:

| 1. Do you smoke cigarattes ???????? | 你抽煙嗎? (有抽煙保費較貴喔) |
| 2. skin disease, including acne | acne 這個字是粉刺的意思，另外一個常用字pimple，是青春痘 |
| 3. frequent or severe headaches | 經常性或嚴重的頭痛 |
| 4. Severe head injury | 嚴重的頭部傷害 |
| 5. Corrective lenses (glasses or contacts) | lense是鏡頭的意思，在這裡指的是眼鏡，glasses是一般我們戴在鼻樑上的眼鏡，contacts是隱形眼鏡（注意是複數） |
| 6. Eye disease | 眼疾 |
| 7. Ear, nose, or throat trouble | 耳鼻喉方面的問題 |
| 8. Hearing difficulty | 聽力困難 |
| 9. Ruptured or perforated ear drum | 耳膜破裂或穿孔 |
| 10. Hay fever or other non-drug allergy | 這個在美國倒是很常見，Hay fever指的是花粉熱 |
| 11. Severe tooth or gum trouble | 嚴重的牙齒或口腔毛病，這裡gum指的不是我們嚼的口香糖(chewing gum)，gum這裡指的是口腔 |
| 12. Thyroid disorder | Thyroid，甲狀腺 |
| 13. Close contact with tuberculosis | 這個字tuberculosis倒是很常出現在Reader's Digest裡，肺結核的意思 |
| 14. Abnormal chest x-ray | 是不是有過異常的x-ray finding |
| 15. Asthma | 氣喘，記得這個字好像曾經出現在托福的聽力裡，托福聽力也常常出現allergy, allergic這些字，因為美國人有hay fever的不在少數 |
| 16. Shortness of breath | 呼吸困難，竟然是用shortness這個字，我就想不到 |
| 17. Chest pain or pressure | 胸痛或胸口鬱悶 |
| 18. Chronic cough | chronic是慢性疾病的意思，不過這裡我想翻譯成「久咳不癒」比較適當 |
| 19. Heart pounding | 心絞痛, pound是重擊. |
| 20. Heart murmur | 心雜音, murmur是喃喃細語的意思. |
| 21. High blood pressure | 高血壓 |
| 22. Severe or recurrent abdominal pain | recurrent是一再複發的意思，這個是問你是不是曾有嚴重的或是一再複發的下腹部疼痛 |
| 23. Stomach or duodenal ulcer | duo這個字首是「十二」的意思，duodenal是十二指腸的意思，唸法是duo-'deen-nal. ? ulcer是潰瘍 |
| 24. Chronic or recurent diarrhea | diarrhea這個字也可以寫成diarrhoea，拉肚子的意思 |
| 25. Gall bladder disease | 膽囊方面的毛病.? (Gall bladder 膽囊, bladder 則指膀胱) |
| 26. Sexually transmitted disease | 性傳染病. 嘿.. 你沒有得到吧. |
| 27. Hernia (rupture) | 疝氣, 據說女生也會得, 而且發生的地方也不只胯下~ |
| 28. Hemorrhoids (piles) | 痔瘡，前面的是醫學名詞，後面的是我們平常用的。像是上小號，正式的名稱是urinate，不過這是醫生說的，我們通常都說piss，或是直接用pee |
| 29. Urinary problem | 知道urinate，這個字可以猜猜看吧？ |
| 30. Blood, sugar, albumin in urine | 尿中是否有糖, 血或是血蛋白...因為正常人尿中這些量都是極少的. albumin是一種血蛋白 |
| 31. Bone or joint disability or deformity | 骨骼或關節失能或變型 |
| 32. Loss of an extremity or portion thereof | extremity是四肢的意思，當然你要是說arms and legs人家也聽得懂啦。這句話是問， 你是不是有截肢過？ |
| 33. "Trick" joint | 經常性的關節脫落 |
| 34. Varicose veins | 靜脈曲張 |
| 35. Foot trouble | 腳的毛病 |
